New patient

If you are attending the clinic for the first time we ask that you arrive 10-15 mins prior to your appointment to complete our registration process. Please ensure you have discussed for possible costs for privately charge item like driving medical, some surgical procedures and etc. 

Arrival at Clinic and Waiting Time

Please present to the reception desk when you arrive at the clinic and after you have finished seeing your doctor. If you have been waiting in the clinic for your appointment for longer than 30 minutes or any concerns about waiting times, please advise reception. If you are unable to attend your appointment, please contact the clinic no less than 2 hours prior to your appointment to cancel. 

Did not Attend (DNA) Policy

 For any appointment that is not attended or not cancelled 2 hours prior to the appointed time will count as DNA. DNA appointment will be charged as your full consultation time as booked with no medical rebate (fully out of pocket) with no exceptions.

 2 hours before the appointment time, you can cancel the appointments via

Telehealth policy

  1. The existing patients who had face-face consultation with our GPs in last 12 months are eligible to have telehealth consultation.

  2.  All telehealth consultations are applied for a private fee. (Regardless of if you hold a valid concession or pension card or a child under 15 years of age).

  3. There are limitations as to what can and cannot be done via telehealth. General Practitioners cannot conduct a physical examination, there is a greater risk of misdiagnosis associated with telehealth services. In the end, some health services cannot be replicated virtually and require in-person visits. 

Private consultations ( non Medicare cover) Policy

Medicare does not rebate for following consultations as it is not related to medical illness. The whole fees will be out of pocket. To compete those services, it is essential to make a standard appointment with a dr first to arrange require assessments such as blood test, hearing test, imaging etc. Some services may complete in one consult, but some may take more than one consultation.

  • MVA (motor vehicle accident) (require long appt for first visit)

S8 drug and drug of dependence policy

 Our practice has drug of dependence policy in place.

           Our practice does not provide opioid or S8 medications on your first visit. 

Reminder System

Ellenbrook Medical Centre will send out reminder for pap smears, immunisations, cervical and bowel cancer screening, health assessments, chronic disease management and other services on a regular basis according to under the state & National Patient Reminder System. You may be contacted us via phone, text message or email as part of our ongoing preventative health recall and remind system. IF you do not wish to receive, please advise our reception staff. 

Recall system

Recall is essential part of comprehensive care. Our practice has recall policy in place. You would need to require to response to see your GP within 24 hours from contact for urgent recall. You will be contacted us via phone, text message or email for non-urgent recalls. You are responsible to attend for your follow-up results and review and further management. It is mandatory and cannot be opted out.  

Transferring Medical Records

We do not charge a fee to send the patient health summary to other health professionals. However, for the whole file transfer, there will be the fees for administration, copying and postage or disc. All the medical record requests on your behalf will only be accepted as written requests that include your written consent. 

Interpreter Service-Translating and Interpreting Service (TIS National)

Please advise when booking your appointment if you require the interpreter service.

After-hour care

Our practice affiliated with Night dr for after-hour care. You can ring 1300 644 483 for after-hour care. If you are experiencing chest pain,

shortness of breath or any other type of pain, please ring 000 and go to emergency.

Privacy

Our clinic complies with the Privacy Amendment (Private Sector) Act 2012 and the Australian privacy Principles. All information collected is treated as “sensitive information”. All patients’ health records and information are handled and stored to protect the privacy of the patient. Our New patient information Form requests patients to sign an agreement for our doctors to discuss a patient’s health care/treatment with other relevant health professionals. Patients always have the right to decline this agreement.
